Responsibilities

As a Revisit Nurse (LPN) at Hartford HealthCare at Home, your responsibilities will include:

• Facilitating communication between the patient and the interdisciplinary team
• Developing and maintaining therapeutic relationships with patients
• Providing patient education and promoting self-advocacy and independence
• Utilizing state-of-the-art technology to document skilled services provided

Requirements

To qualify for this role, you will need:

• Valid CT LPN license
• At least one year of LPN experience, homecare experience preferred
• Valid driver's license and reliable transportation
• Computer proficiency
