As a Care Transition Nurse, you will collaborate with RNs, social workers, skilled nursing facilities, assisted living facilities, independent living facilities, home care agencies, and physicians. Your role will involve providing education to customers, patients, and families, ensuring safe and effective transitions for patients moving from one level of care to another. By serving as a bridge between the healthcare team and the patient/caregiver, you will facilitate better communication, reduce facility re-admissions, and support enhanced self-management skills for patients.
Key Responsibilities
- Coordinate and facilitate transitions of care for patients across the post-acute care continuum
- Provide education and guidance to patients and caregivers
- Support effective communication between patients and the healthcare team
- Conduct home visits as needed
Requirements
- BSN Required
- Current RN License in the state of Connecticut
- Three years+ nursing experience
- Active Driver's License and reliable transportation
- Computer literate with the ability to learn new applications
- Comfortable with regional travel and home visits
- Excellent written and verbal communication skills
- Public speaking skills for educational programs
- Self-starter
