Position Overview:

As a Revisit Nurse (LPN), you will play a vital role in providing integrated support to our patients. You will facilitate communication between patients and the interdisciplinary team, develop therapeutic relationships, and utilize state-of-the-art technology to document skilled services provided. Your expertise and dedication will contribute to the overall well-being and independence of our patients.

Job Responsibilities:

• Facilitate communication between the patient and the interdisciplinary team
• Develop and maintain therapeutic relationships
• Provide education to promote self-advocacy and independence
• Utilize state-of-the-art technology for documentation

Qualifications:

• Valid CT LPN license
• Minimum one year of LPN experience (Homecare experience preferred)
• Valid driver's license and reliable transportation
• Computer proficiency
