What You Will Do

As a Revisit LPN, you will play a vital role in providing personalized care to our patients. Your responsibilities will include:

• Facilitating communication between the patient and the interdisciplinary team
• Developing and maintaining a therapeutic relationship by providing education and promoting self-advocacy and independence
• Utilizing state-of-the-art technology to document skilled services provided

Qualifications

To be successful in this role, you will need:

• Valid CT LPN license
• One year of LPN experience; Homecare experience preferred
• Valid driver's license and reliable transportation
• Computer proficiency
