Job Responsibilities

• Conduct comprehensive assessments with patients to gather relevant information and establish treatment plans
• Provide individual, family, and group therapy services using evidence-based practices
• Assess and manage risk for patients with high-risk behaviors and symptoms
• Document assessment outcomes, treatment plans, progress notes, and case management activities
• Coordinate care with other healthcare providers, state agencies, and community organizations
• Stay up to date with required continuing education, licensing requirements, and quality assurance processes

Qualifications

• Connecticut Licensed MSW or Master's degree with current Connecticut certification as an Alcohol/Drug Counselor (CADC)
• Excellent communication skills
• Knowledge and behaviors to meet age-specific patient needs
• Bilingual preferred
