Summary

Hartford HealthCare is seeking a skilled and dedicated Nurse Practitioner or Physician Assistant to join our REACH (Resource for Elderly And Care at Home) Program. In this role, you will provide comprehensive care to our home-bound patients in the greater Hartford area. Responsibilities

As a Nurse Practitioner or Physician Assistant in our REACH Program, your responsibilities will include:

  • Conducting home visits to assess and provide medical care to geriatric patients
  • Collaborating with a multidisciplinary team to develop comprehensive care plans
  • Ordering and interpreting diagnostic tests
  • Prescribing medication and monitoring treatment effectiveness
  • Providing health education and counseling to patients and their families

Qualifications

To be successful in this role, you should have the following qualifications:

  • Unrestricted State of Connecticut Medical License (or eligible)
  • APRN (Family, Adult/Geri Nurse Practitioner) or PA
  • Minimum of two years of post-graduate experience as a Nurse Practitioner or Physician Assistant
