Hartford HealthCare is seeking a motivated and dedicated Nurse Practitioner (APRN) or Physician Assistant (PA) to collaborate with a high-functioning team of providers. In this role, you will primarily provide follow-ups with patients, assessing and managing their endocrinology conditions. You will also work closely with our team on diabetes wellness and prevention initiatives.

Key Responsibilities:

• Collaborate with providers to deliver comprehensive care to patients with complex endocrinology disorders
• Conduct follow-up appointments and assess patients' conditions
• Evaluate test results, prescribe medications, and provide appropriate treatments
• Educate patients on diabetes management, wellness, and prevention
• Comply with all clinical documentation and billing requirements

Qualifications:

• Eligibility to be licensed in Connecticut as a Nurse Practitioner (APRN) or Physician Assistant (PA)
• Certification as a Family Nurse Practitioner (FNP), Adult Nurse Practitioner (ANP), or Adult Gerontology Primary Care Nurse Practitioner (AGPCNP) is required
• Previous experience in endocrinology/diabetes as an APRN, PA, clinical student, or RN
