Responsibilities

As the Clinical Team Manager, you will:

• Manage, develop, and mentor clinical staff and ensure their professional development needs are met
• Maintain compliance with regulatory agency policies and procedures
• Be accountable for team performance and ensure desired clinical and operational outcomes are achieved
• Utilize clinical software to identify opportunities for education, optimize service utilization, and track team performance
• Resolve patient care problems collaboratively with internal and external stakeholders
• Oversee daily assignments for clinical and clerical staff
• Determine staffing needs and coordinate hiring to meet service demands
• Participate in preceptor programs
• Anticipate customer needs and respond promptly and courteously

Qualifications

To be successful in this role, you should have:

• Nursing qualifications: Registered Nurse (RN) with a license to practice in the State of Connecticut
• Education: According to current state requirements
• Experience: According to current state licensure regulations, with OASIS and chronic care management certification

Alternatively,

• Therapy qualifications: Physical Therapist, Occupational Therapist, or Speech Therapist with a license to practice in the State of Connecticut
• Education: Bachelor's degree in one of the therapy disciplines
• Experience: Minimum of 5 years clinical experience, with home health experience and prior supervisory experience preferred, along with OASIS and case management certification
