Responsibilities

As a Revisit Nurse (LPN), you will have the opportunity to provide personalized care to our patients in a rewarding environment. Your responsibilities will include:

• Facilitating communication between the patient and the interdisciplinary team
• Developing and maintaining a therapeutic relationship by providing education and promoting self-advocacy and independence
• Utilizing state-of-the-art technology to document skilled services provided

Qualifications

To qualify for this position, you must have:

• Valid CT LPN license
• Minimum of one year of LPN experience; homecare experience preferred
• Valid driver's license and reliable transportation
• Computer proficiency
