Job Responsibilities
- Provide direct client care using individual, group, and family therapy tailored to each client's needs
- Prepare accurate medical documentation to meet regulatory requirements
- Assess and plan care, prioritizing treatment and identifying appropriate resources
- Deliver in-home clinical services and crisis intervention in multiple locations
- Educate clients and family members, facilitate referrals, and support the recovery process
- Collaborate effectively across programs for clients with mental health and substance use disorders
- Negotiate level of care and communicate changes in client's status to third-party payers
- Work collaboratively with internal and external partners for integrated healthcare delivery
- Conduct emergency evaluations as required
- Participate in team meetings and staff education
- Assume clinical oversight of daily operations in the absence of the Program Manager
Qualifications
- Master's Degree in Social Work, Counseling, or related field
- Minimum of three years' direct services experience with psychiatric and substance abuse populations
- Previous experience in assessment, diagnosis, individual/group therapy, and crisis intervention
- Valid CT driver's license
- Active Connecticut LCSW, LPC, LADC, or LMFT license
