Job Responsibilities

• Provide direct client care using individual, group, and family therapy tailored to each client's needs
• Prepare accurate medical documentation to meet regulatory requirements
• Assess and plan care, prioritizing treatment and identifying appropriate resources
• Deliver in-home clinical services and crisis intervention in multiple locations
• Educate clients and family members, facilitate referrals, and support the recovery process
• Collaborate effectively across programs for clients with mental health and substance use disorders
• Negotiate level of care and communicate changes in client's status to third-party payers
• Work collaboratively with internal and external partners for integrated healthcare delivery
• Conduct emergency evaluations as required
• Participate in team meetings and staff education
• Assume clinical oversight of daily operations in the absence of the Program Manager

Qualifications

• Master's Degree in Social Work, Counseling, or related field
• Minimum of three years' direct services experience with psychiatric and substance abuse populations
• Previous experience in assessment, diagnosis, individual/group therapy, and crisis intervention
• Valid CT driver's license
• Active Connecticut LCSW, LPC, LADC, or LMFT license
