Revisit Nurse (LPN) - HomeCare

In this role, you will facilitate communication between patients and the interdisciplinary care team, develop therapeutic relationships, and provide education to promote self-advocacy and independence. You will utilize state-of-the-art technology to document skilled services provided. Responsibilities

• Facilitate communication between patients and interdisciplinary care team

Qualifications

• Licensed Practical Nurse with a valid CT LPN license
• Minimum of 1 year LPN experience; Homecare experience preferred
• Valid driver's license and reliable transportation
• Computer proficiency
