Summary

Hartford HealthCare is seeking a talented and experienced Quality & Safety Clinical Nurse Manager (RN) to join our HomeCare team. In this role, you will have the opportunity to lead a team responsible for managing the quality improvement and patient safety processes in local settings. You will be a subject matter expert in clinical nursing practice, quality, and the management of complex conditions.

Responsibilities

  • Use coaching methodology with individual staff, teams, and large staff groups to incorporate clinical core competencies and improve care
  • Integrate evidence-based practice and evidence-based management in care delivery to drive quality improvement
  • Support the Quality Safety Clinicians, coordinators, clinicians, and Regional Directors in local teams
  • Utilize quality improvement tools to assess care, such as apparent-cause analysis, failure modes and effects analysis, and plan-do-study-act
  • Develop processes and interventions to reduce errors during handoffs and workarounds
  • Coach staff on the use of the SBAR process to improve communication and reduce errors
  • Incorporate patient rounds to improve patient-centered care and involve patients and families in reducing errors
  • Understand the connection between quality improvement and costs and develop changes to reduce hospital-acquired complications and costs
  • Collaborate and coordinate with organization-wide quality improvement staff to monitor care delivery and meet regulatory requirements
  • Utilize interprofessional teams effectively to achieve improvement goals
  • Provide quality care when delivering direct patient care
  • Conduct research and propose evidence-based changes in work processes to support an improved care environment
  • Support patient risk management and prioritize patient safety by identifying causes and solutions to reduce injuries

Qualifications

Education:

  • Bachelor's Degree in Nursing

Experience:

  • Minimum 2 years of experience in Home Health Care

Licensure, Certification, Registration:

  • Current CT State RN License
