Summary:

Hartford HealthCare is seeking a skilled and compassionate Nurse Practitioner (APRN) or Physician Assistant (PA) to join our multi-provider, adult primary care practice in Manchester, Connecticut. As part of our team, you will have the opportunity to provide exceptional care to our patients in an outpatient setting. As a Nurse Practitioner or Physician Assistant, you will have the support of our dedicated team and the latest clinical and informatics technology to enhance patient outcomes and streamline care. With a flexible schedule and no call responsibilities, you can achieve a healthy work-life balance.

Responsibilities:

• Perform routine physical examinations and assessments
• Formulate medical, educational, and psychosocial plans to treat patients
• Collaborate with physicians and care team members to develop plans for further evaluation and treatment
• Provide appropriate treatment within the scope of your practice
• Identify and refer patients to specialists as necessary

Requirements:

• Unrestricted State of Connecticut Medical License (or eligible)
• APRN (Family, Adult/Geri Nurse Practitioner) or PA certification
• New graduates are welcome to apply
• Preferred: At least two years of post-graduate experience as an APRN or PA
