Summary

Hartford HealthCare is currently seeking a dedicated and experienced Physician Assistant or Nurse Practitioner (APRN) to join our top Cardiology practice in Fairfield County, Connecticut. In this role, you will provide direct care to patients in an outpatient setting in collaboration with our physicians. We are looking for someone who is passionate about cardiology, able to deliver high-quality patient-centered care, and has excellent interpersonal and organizational skills.

Role Responsibilities

  • Provide high-quality patient-centered care, education, and advocacy for patients and their families
  • Diagnose, treat, and manage complex cardiovascular care, including acute and chronic diseases, medication management, and other therapies
  • Collaborate with physicians and the healthcare team to provide excellent patient care
  • Effectively communicate with patients, family members, and healthcare professionals
  • Educate patients and their families on medical conditions, treatment regimens, and expected outcomes

Qualifications

To qualify for this position, you must have:

  • Graduate of an accredited Physician Assistant or Nurse Practitioner Master’s Program
  • Licensure, or eligibility to be licensed, as a Physician Assistant (PA-C), Family Nurse Practitioner (FNP), or Adult Geriatric Primary Care (AGPCNP) in the state of Connecticut
