Synchrony Health Services, a subsidiary of Trilogy Health Services LLC, is seeking a licensed Physical Therapy Assistant to join our team at Bostonian Skilled Nursing and Rehab Center. We are a dynamic and innovative Skilled Nursing and Assisted Living health campus located in Dorchester, MA. As a Physical Therapy Assistant, you will play a pivotal role in assisting our patients in their journey to recovery and improved mobility. You will work under the supervision of a licensed Physical Therapist and will collaborate with a multidisciplinary team to provide comprehensive and compassionate care.

Responsibilities

• Provide physical therapy treatments and interventions as directed by the licensed Physical Therapist
• Assist patients in performing therapeutic exercises and activities
• Monitor patient progress and document treatment outcomes
• Educate patients and their families on proper techniques for exercises and activities
• Collaborate with the healthcare team to develop effective patient care plans
• Maintain accurate and up-to-date patient records

Qualifications

• Must have an active or pending Physical Therapy Assistant (PTA) state license
• Proven track record of successful patient care experience
• Experience working with a geriatric population preferred
