Responsibilities: As a Physical Therapist (PT) with Hartford HealthCare at Home, you will be responsible for evaluating, planning, and implementing treatment for homebound patients referred for physical therapy. Your role will involve evaluating patients, determining treatment strategies, and delivering or delegating services to maximize functional independence. You will focus on addressing physical dysfunction, pain, strength, mobility, balance, coordination, endurance, and functional abilities.

Qualifications:

• PT experience required
• Graduate of a Physical Therapy Program from an accredited American Physical Therapy Association school
• Active CT PT license
• CPR certification required
