Responsibilities

As a Physical Therapist, you will be responsible for evaluating, planning, and implementing treatment for homebound patients referred for physical therapy. You will use a variety of techniques and exercise programs to help patients improve their physical function and regain independence. Responsibilities include:

• Evaluating patients and developing individualized treatment plans
• Administering physical therapy interventions
• Providing education and instructions to patients and their families
• Monitoring and documenting patient progress
• Collaborating with other healthcare professionals

Qualifications

• PT experience required
• Graduate of an accredited Physical Therapy Program
• Active CT PT license
• CPR certification required
