Hartford HealthCare at Home is seeking a skilled and compassionate Physical Therapist (PT) to join our dedicated team. As a Physical Therapist at Hartford HealthCare, you'll have the opportunity to work in homecare, providing care to patients in the Southington and/or Meriden area. We also have opportunities available in other regions, such as Farmington/Plainville, North Haven, and Norwich/Mansfield. Job Responsibilities

As a Physical Therapist at Hartford HealthCare at Home, your responsibilities will include:

• Evaluating and developing treatment plans for homebound patients referred for physical therapy
• Independently delivering or delegating services to identify, prevent, remediate, and rehabilitate physical dysfunction or pain
• Utilizing a variety of evaluation techniques and therapeutic interventions to enhance patients' functional independence
• Assessing areas such as strength, joint mobility, soft tissue integrity, balance, coordination, posture, and functional abilities

Qualifications

To qualify for this position, you'll need:

• PT experience
• Graduation from an accredited Physical Therapy Program
• Active CT PT license
• CPR certification
