Physical Therapist - Homecare

Hartford HealthCare is seeking a Physical Therapist (PT) to join our team in providing quality care to patients in the comfort of their own homes. Responsibilities:

• Evaluate, plan, and implement treatment for homebound patients referred for physical therapy
• Independently evaluate patients and develop treatment strategies to prevent, remediate, and rehabilitate physical dysfunction or pain
• Utilize a variety of techniques, exercises, and therapeutic procedures to maximize patient's functional independence

Qualifications:

• Minimum of 2 years of PT experience
• Graduate of an accredited Physical Therapy Program
• Active Connecticut PT license
• CPR certification
