Responsibilities

• Supervise supportive personnel in the pharmacy department to ensure safe and accurate preparation and dispensing of medications
• Design drug therapy that is appropriate, correct, and cost-effective to achieve positive patient outcomes
• Provide direct patient care for pain management according to physician's request
• Identify, prevent, and resolve adverse drug events to maintain patient safety
• Participate in Health Care Team activities to provide valuable drug information and facilitate patient care
• Investigate and resolve drug and pharmacy-related problems
• Document interventions and outcomes to track pharmacy activity and quality improvement
• Demonstrate competencies to meet age-specific patient needs
• Function as a charge pharmacist when required

Qualifications

• Pharm.D. required
• Current CT Pharmacist license or eligible within 45 days of hire
• PGY-1 Pharmacy residency preferred
• Prior experience with EPIC systems preferred
