Responsibilities
As a Pharmacist, you will:
- Supervise the work of supportive personnel including pharmacy technicians and associates, ensuring safe and accurate preparation and dispensing of medications in compliance with regulations.
- Optimize drug therapy for patients by designing appropriate, correct, and cost-effective treatment plans.
- Provide direct patient care for pain management, including evaluation, recommendations, and follow-ups.
- Maintain safety of drug therapy through screening mechanisms and collaboration with healthcare providers and patients to identify and prevent adverse drug events.
- Serve as a liaison between the healthcare team and the pharmacy, providing valuable drug information and facilitating effective patient care.
- Investigate and resolve drug and pharmacy-related problems through studies and problem-solving techniques.
- Document interventions, outcomes, and statistics to track activity, success, and opportunities for improvement.
- Demonstrate competencies and behaviors to meet age-specific patient needs.
- Function as a charge pharmacist on occasion.
Qualifications
To be considered for this role, you need:
- Pharm.D. degree
- Current CT Pharmacist license or eligible within 45 days of hire
- PGY-1 Pharmacy residency preferred
- Prior experience with EPIC systems preferred
