Responsibilities

• Perform comprehensive OASIS assessments to establish individualized care plans
• Facilitate communication between patients and the interdisciplinary team
• Develop and maintain therapeutic relationships by providing education and promoting self-advocacy and independence
• Utilize state-of-the-art technology to document skilled services provided

Requirements

• Registered Nurse with a valid CT RN license
• Minimum of one year of RN experience; Homecare experience preferred
• Valid driver's license and reliable transportation required
• Computer proficiency required
