Responsibilities

• Conduct in-home primary care visits for elderly patients
• Perform routine physical examinations
• Formulate medical, educational, and psychosocial plans to treat patients
• Collaborate with physicians and care team members to develop comprehensive treatment plans

Requirements

• Unrestricted State of Connecticut Medical License (or eligible)
• APRN (Family, Adult/Geri Nurse Practitioner) or PA
• Minimum of two years of post-graduate experience
