Hartford HealthCare is seeking a highly skilled and experienced Nurse Practitioner or Physician Assistant to join our REACH (Resource for Elderly And Care at Home) Program. As a member of our geriatric care team, you will provide home-based care for our elderly patients in the greater Willimantic area. This is a full-time position with a weekday schedule. Nurse Practitioners and Physician Assistants Role Scope
As a Nurse Practitioner or Physician Assistant in our REACH Program, you will perform routine physical examinations, formulate medical, educational, and psychosocial plans to treat patients, and collaborate with the physician(s) and care team members. You will have the support of the Office of Advanced Practice and access to resources that promote your professional growth and success.
Qualifications
- Licensure: Unrestricted State of Connecticut Medical License (or eligible)
- Training: APRN (Family, Adult/Geri Nurse Practitioner) or PA
- Experience: At least two years of post-graduate experience as a Nurse Practitioner or Physician Assistant
