Summary

Hartford HealthCare is looking for a compassionate and experienced Nurse Practitioner or Physician Assistant to join our team in providing home-based geriatric care. As a part of our REACH (Resource for Elderly And Care at Home) Program, you will have the opportunity to serve our geriatric patients in the greater Willimantic area. This is a full-time position offering competitive compensation, comprehensive benefits, and a supportive work environment.

  • Job Schedule: Monday to Friday
  • Job Shift: Shift 1 (Weekdays)
  • Location: Homecare based in the greater Willimantic, CT area

Responsibilities

As a Nurse Practitioner or Physician Assistant in our geriatric homecare program, you will:

  • Conduct routine physical examinations
  • Formulate medical, educational, and psychosocial plans to treat patients
  • Develop plans for further evaluation and referral to specialists, when necessary
  • Provide appropriate treatment within the scope of your practice

Qualifications

To be considered for this position, you must have:

  • Unrestricted State of Connecticut Medical License (or eligible)
  • APRN (Family, Adult/Geri Nurse Practitioner) or PA training
  • At least two years of post-graduate experience as a Nurse Practitioner or Physician Assistant
