Summary
Hartford HealthCare is looking for a compassionate and experienced Nurse Practitioner or Physician Assistant to join our team in providing home-based geriatric care. As a part of our REACH (Resource for Elderly And Care at Home) Program, you will have the opportunity to serve our geriatric patients in the greater Willimantic area. Responsibilities
As a Nurse Practitioner or Physician Assistant in our geriatric homecare program, you will:
- Conduct routine physical examinations
- Formulate medical, educational, and psychosocial plans to treat patients
- Develop plans for further evaluation and referral to specialists, when necessary
- Provide appropriate treatment within the scope of your practice
Qualifications
To be considered for this position, you must have:
- Unrestricted State of Connecticut Medical License (or eligible)
- APRN (Family, Adult/Geri Nurse Practitioner) or PA training
- At least two years of post-graduate experience as a Nurse Practitioner or Physician Assistant
