Summary

In this role, you will collaborate with a high-functioning team of providers to primarily provide follow-up care to patients with complex endocrinology disorders, with a particular focus on diabetes management. Responsibilities

  • Collaborate with a team of providers to provide high-quality, patient-centered care to individuals with complex endocrinology disorders.
  • Focus on follow-up care for patients with a primary focus on diabetes management.
  • Utilize tailored Endocrinology EPIC templates for efficient documentation and workflow.
  • Spend 90% of time in the outpatient department and 10% in the inpatient setting performing diabetes consults.
  • Work collaboratively with other healthcare professionals to ensure coordinated care.
  • Actively participate in ongoing professional development and stay up-to-date with the latest advancements in endocrinology.

Requirements

  • Connecticut state licensure as a Nurse Practitioner (APRN) or Physician Assistant (PA-C).
  • National Board Certification as a Family Nurse Practitioner (FNP), Adult Nurse Practitioner (ANP), or Adult Gerontology Primary Care Nurse Practitioner (AGPCNP) is preferred.
  • Experience in endocrinology and diabetes management is highly desirable.
