Responsibilities

• Coordinate, negotiate, communicate, and manage the care of assigned patient population along a continuum
• Facilitate achievement of optimal patient outcomes within appropriate length of stay (LOS) and resource utilization parameters

Requirements

• Education: Graduate of a school of nursing; BSN or MSN preferred
• Experience: Two (2) to Three (3) years recent clinical nursing experience in an acute care setting
• License and Certification: Current unrestricted License in good standing to practice as a Registered Nurse in the State of Connecticut; Current Basic Life Support (BLS) certification; Ability to obtain and maintain department-specific certifications; Certified Nurse Care Manager preferred
