Responsibilities: As a Licensed Practical Nurse (LPN) at Hartford HealthCare at Home, you will:

• Facilitate communication between the patient and interdisciplinary team
• Develop and maintain a therapeutic relationship with the patient, providing education and promoting self-advocacy and independence
• Utilize state-of-the-art technology to document skilled services provided

Requirements:

• Valid CT LPN license
• Minimum of one year of LPN experience, with homecare experience preferred
• Valid driver's license and reliable transportation
• Computer proficiency required
