Responsibilities

As a Licensed Practical Nurse with Hartford HealthCare, your responsibilities will include:

• Facilitating communication between the patient and the interdisciplinary team
• Developing and maintaining a therapeutic relationship with the patient
• Providing education and promoting self-advocacy and independence
• Utilizing state-of-the-art technology to document skilled services provided

Requirements

To be successful in this role, you will need:

• Valid CT LPN license
• Minimum of one year of LPN experience (Homecare experience preferred)
• Valid driver's license and reliable transportation
• Computer proficiency
