Role

As an LPN in Primary Care, you will play a vital role in delivering quality and cost-effective direct care to patients and their families. Collaborating with a multidisciplinary healthcare team, you will assist patients in achieving their established outcomes of care. Your responsibilities will include:

• Provide direct patient care and perform various nursing procedures.
• Administer medications and treatments as prescribed by healthcare providers.
• Collaborate with the healthcare team to develop and implement care plans.
• Educate patients and families about their health conditions and self-care.
• Ensure accurate documentation of patient assessments, interventions, and outcomes.

Qualifications

To be successful in this role, you should have the following qualifications:

• Current Connecticut Licensed Practical Nurse (LPN) license.
• Previous experience in a Medical/Surgical Office setting preferred.
