Summary:

Hartford HealthCare, one of Connecticut’s most comprehensive healthcare networks, is seeking a Licensed Practical Nurse (LPN) to join their Behavioral Health team. This is a part-time position with a standard shift of 32 hours per week, working from 11:00 PM to 7:30 AM, Wednesday through Saturday. As an LPN in the Behavioral Health department, you will provide timely, effective, and high-quality nursing collaboration for all enrollees in the program under the direct supervision of a registered nurse. Responsibilities:

  • Collaborate with the registered nurse and other healthcare professionals to provide coordinated care and support for the program enrollees
  • Conduct individual and group education sessions on chronic conditions, newly diagnosed conditions, medications, health promotion, wellness, and prevention
  • Assist in coordinating care with other medical providers and facilities as necessary
  • Work closely with the Behavioral Health Home (BHH) team to ensure comprehensive and patient-centered care
  • Adhere to all nursing standards, protocols, and policies

Requirements:

  • Completion of a licensed LPN program
  • Valid CT LPN license
  • Minimum of one year of experience in Medical/Surgical, Critical Care, Psychiatric, or Substance Abuse Treatment preferred
  • CPR certification within six months of employment
