Hartford HealthCare at Home is seeking compassionate Certified Nursing Assistants (CNA) and Aides to join our HomeCare team in Plymouth, CT. As a CNA, you will provide essential personal care and housekeeping assistance to our clients in the comfort of their own homes.

Responsibilities

• Provide personal care and housekeeping assistance to clients in their homes, following the established plan of care
• Assist with personal hygiene, grooming, and meal preparation
• Report changes in clients' condition to the client care coordinator or supervisor
• Perform routine housekeeping and home management duties
• Record each client's care on the care plan and submit in a timely manner
• Respond sensitively to the complex needs of a terminal client, providing end-of-life care
• Assist with routine therapy exercises
• Participate in case conferences as requested

Qualifications

• Education: High school diploma or equivalent preferred
• Licensure/Certification: CNA Certificate approved by the Connecticut Department of Public Health and Addiction Services
• Experience: Six months experience as a Home Care Aide/CNA in a home care agency preferred
