Hartford HealthCare is seeking a compassionate and dedicated Certified Nursing Assistant (CNA) to join our HomeCare team. As a CNA, you will provide personal care and housekeeping assistance to our clients in their homes, following the established care plan under the direction of the client care coordinator. You will play a critical role in helping our clients maintain their independence and overall well-being.

- Job Shift: Shift 1
- Shift Details: Monday through Friday, Weekends and Holidays as needed
Responsibilities
As a CNA with Hartford HealthCare, your responsibilities will include:
- Providing personal care and housekeeping assistance to our clients
- Assisting with personal hygiene and grooming care
- Preparing and serving meals based on clients' nutritional needs
- Reporting any changes in clients' condition to the client care coordinator or supervisor
- Performing routine housekeeping and home management duties
- Documenting and submitting care plan records in a timely manner
- Responding sensitively to the complex needs of terminal clients, including providing end-of-life care
- Assisting with routine therapy exercises
- Participating in case conferences as requested
Qualifications
To qualify for this role, you will need:
- Education: High School diploma or equivalent (preferred)
- Licensure/Certification: CNA Certificate approved by the Connecticut Department of Public Health and Addiction Services
- Experience: Six months experience as a Home Care Aide/CNA in a home care agency (preferred)
