Summary

Hartford HealthCare is seeking a dedicated Care Transition Nurse (RN) to join our Center for Health Aging on a Per Diem basis. In this role, you will work in collaboration with RNs, social workers, skilled nursing facilities, assisted living facilities, independent living facilities, home care agencies, and physicians to provide education and coordinate the care of patients transitioning across the post-acute care continuum. Responsibilities

• Serve as a bridge between the healthcare team and the patient/caregiver
• Coordinate the care of patients moving from one level of care to another
• Educate customers, patients, and families on effective care transitions and self-management skills
• Support enhanced communication between the patient and the healthcare team
• Conduct home visits throughout the state

Requirements

• BSN Required
• Current RN License in the state of Connecticut
• Minimum three years of nursing experience
• Active Driver's License & reliable transportation
• Computer literate with ability to learn new applications
• Comfortable with regional travel and home visits
• Excellent written and verbal communication skills
• Public speaking skills for educational programs
• Self-starter
