Summary

Hartford HealthCare is looking for a Care Transition Nurse (RN) to join our Center for Health Aging team on a Per Diem basis in Hartford, CT. In this role, you will work closely with RNs, social workers, skilled nursing facilities, assisted living facilities, independent living facilities, home care agencies, and physicians to coordinate the care of patients moving from one level of care to another. You will serve as a bridge between the healthcare team and patients or caregivers, ensuring safe and effective transitions across the post-acute care continuum.

• Shift Details: Per Diem Day Hours - Mon-Fri, covering vacation staff as needed

Responsibilities

• Collaborate with RNs and social workers to coordinate care transitions for patients
• Educate patients, families, and caregivers on care transition processes
• Bridge communication between healthcare team and patients or caregivers
• Reduce facility re-admissions through effective care transitions
• Conduct home visits throughout the state

Requirements

• BSN required
• Current RN license in the state of Connecticut
• Minimum of three years of nursing experience
• Active driver's license and reliable transportation
• Computer literate with the ability to learn new applications
• Comfortable with regional travel and home visits
• Excellent written and verbal communication skills
• Strong public speaking skills for educational programs
• Self-starter with strong organizational skills
