• Shift Details: Per Diem Day Hours - Mon-Fri, covering vacation staff as needed

Responsibilities:

• Collaborate with RNs, social workers, and various healthcare facilities to coordinate patient care during transitions from one level of care to another
• Educate patients, families, and caregivers on effective care transitions, self-management skills, and disease processes
• Serve as a bridge between the healthcare team and patients/caregivers, facilitating communication and reducing facility re-admissions
• Conduct home visits across the state to ensure comprehensive care

Requirements:

• BSN degree required
• Current RN license in the state of Connecticut
• Minimum of three years of nursing experience
• Active driver's license and reliable transportation
• Strong computer literacy and willingness to learn new applications
• Comfortable with regional travel and home visits
• Excellent written and verbal communication skills
• Proficient in public speaking for educational programs
• Self-motivated and able to work independently
