Hartford HealthCare is seeking a dedicated and experienced Care Transition Nurse (RN) to join our Center for Health Aging team on a per diem basis. As a Care Transition Nurse, you will play a crucial role in coordinating the care of patients moving from one level of care to another, ensuring a safe and effective patient transition across the post-acute care continuum. Responsibilities

• Collaborate with RNs, social workers, skilled nursing facilities, assisted living facilities, independent living facilities, home care agencies, and physicians to coordinate the care of patients transitioning between levels of care
• Provide education to customers, patients, and families to facilitate effective care transitions and improve self-management skills
• Serve as a bridge between the healthcare team and the patient/caregivers, ensuring clear communication and reduced facility re-admissions
• Conduct home visits across the state of Connecticut

Qualifications

• BSN Required
• Current RN License in the state of Connecticut
• Three years+ nursing experience
• Active Driver's License & reliable transportation
• Computer literate with the ability to learn new applications
• Comfortable with regional travel and home visits
• Excellent written and verbal communication skills
• Public speaking skills for educational programs
• Self-starter
