About the Center for Health Aging

The Hartford HealthCare Center for Healthy Aging is a resource and assessment center that aims to improve the quality of life for seniors, their loved ones, and caregivers. As a Care Transition Nurse, you will work collaboratively with RNs, social workers, skilled nursing facilities, assisted living facilities, independent living facilities, home care agencies, and physicians to educate patients, families, and customers about the care transition process. Your role will be crucial in reducing readmissions and ensuring effective communication among the healthcare team, patients, and caregivers.

Responsibilities

  • Coordinate the care of patients transitioning from one level of care to another
  • Provide education to patients, families, and customers on care transition process
  • Serve as a bridge between the healthcare team and patients/caregivers
  • Support patients in improving self-management skills and knowledge of their illness or disease process
  • Conduct home visits across the state

Requirements

  • Bachelor of Science in Nursing (BSN) required
  • Current RN license in the state of Connecticut
  • Minimum of three years of nursing experience
  • Active driver's license and reliable transportation
  • Comfortable with regional travel and home visits
  • Excellent written and verbal communication skills
  • Strong public speaking skills for educational programs
  • Self-starter with the ability to work independently
