Hartford HealthCare is seeking a compassionate and dedicated Care Transition Nurse (RN) to join our Center for Health Aging team in Durham, CT. As a Care Transition Nurse, you will play a crucial role in coordinating the care of patients as they move from one level of care to another, ensuring a safe and effective transition across the post-acute care continuum.

• Shift Details: Per Diem Day Hours - Mon-Fri (Covering vacation staff as needed)

Responsibilities:

• Work collaboratively with RNs, social workers, skilled nursing facilities, assisted living facilities, independent living facilities, home care agencies, and physicians to provide education and support in coordinating patient care
• Act as a bridge between the healthcare team and the patient/caregivers, ensuring effective communication and reducing facility re-admissions
• Conduct home visits state-wide to assess patient needs and provide guidance on self-management skills and illness/disease processes

Requirements:

• BSN required
• Current RN license in the state of Connecticut
• Minimum of three years of nursing experience
• Active driver's license and reliable transportation
• Proficiency in computer applications and ability to learn new applications
• Comfortable with regional travel and home visits
• Excellent written and verbal communication skills
• Public speaking skills for educational programs
• Self-starter with strong organizational and time management skills
