As a Care Transition Nurse, you will play a crucial role in coordinating the care and ensuring a safe and effective transition of patients across the post-acute care continuum. As a Care Transition Nurse, you will have the opportunity to work in collaboration with RNs, social workers, skilled nursing facilities, assisted living facilities, independent living facilities, home care agencies, and physicians. Your role will involve providing education and support to patients and families, ensuring a smooth transition as they move from one level of care to another.

Responsibilities

• Serve as a bridge between the healthcare team and patients/caregivers
• Coordinate the care of patients moving from one level of care to another
• Provide education to patients, families, and healthcare providers
• Help reduce facility re-admissions
• Support effective care transitions and self-management skills
• Conduct home visits statewide

Requirements

• Bachelor of Science in Nursing (BSN) required
• Current RN license in the state of Connecticut
• Minimum of three years of nursing experience
• Active driver's license and reliable transportation
• Computer literate with the ability to learn new applications
• Comfortable with regional travel and home visits
• Excellent written and verbal communication skills
• Strong public speaking skills for educational programs
• Self-starter
