- Shift Details: Monday - Friday, 7:00 a.m. - 3:00 p.m.
Responsibilities
In this role, you will collaborate with patients, nurses, social workers, physicians, and other healthcare team members to ensure optimal health, access to care, and appropriate utilization of resources. Your responsibilities will include:
- Utilizing the Social Work Process to determine and facilitate the most appropriate level of care and discharge needs.
- Conducting patient assessments and planning, including psychosocial, financial, and discharge planning.
- Collaborating with physicians, nursing, and other members of the healthcare team to fulfill the treatment plan in the highest quality and cost-effective manner.
- Ensuring customer satisfaction by maintaining patient confidentiality and being sensitive to the age and cultural-specific needs of patients.
Qualifications
- Masters in Social Work from an accredited school of Social Work
- Active Licensed Clinical Social Worker (LCSW) licensure in the State of Connecticut
- Minimum of 3 years of Medical Social Work experience
- Acute Care experience preferred
- Case Management certification ACM or CCM strongly preferred
- Strong communication and interpersonal skills
- Knowledge of the Social Work Process
- Ability to manage multiple priorities and projects from initiation to completion
- Ability to effectively utilize a variety of Microsoft Office, Outlook, and Meditech HCIS software
- Skilled in Motivational Interviewing
