About the Role:

As a Care Manager, you will be part of a collaborative practice model that includes patients, nurses, social workers, physicians, healthcare team members, caregivers, and the community. Your role will involve effective resource coordination, communication, and facilitation of care along a continuum. You will assess patient needs, develop a comprehensive care plan, and ensure appropriate utilization of resources while balancing the patient's right to self-determination.

Responsibilities:

  • Utilize your education, experience, and medical social work skills to determine the most appropriate level of care and identify discharge needs.
  • Collaborate with physicians, nursing staff, and healthcare team members to develop and fulfill the treatment plan.
  • Maintain patient confidentiality, ensure customer satisfaction, and adapt to the age and cultural needs of each patient.
  • Support patients and families in accessing care and provide information about available resources.
  • Communicate effectively with interdisciplinary team members, patients, and their support systems.

Requirements:

  • Master's degree in Social Work from an accredited school of Social Work.
  • Active Licensed Clinical Social Worker (LCSW) licensure in the State of Connecticut.
  • Minimum of 3 years of Medical Social Work experience, with acute care experience preferred.
  • Case Management certification (ACM or CCM) strongly preferred.
  • Strong communication and interpersonal skills.
  • Knowledge of the Social Work Process and ability to manage multiple priorities and projects.
  • Proficiency in Microsoft Office, Outlook, Meditech HCIS software, Morrisey, and web-based programs.
  • Skilled in Motivational Interviewing.
