Summary: Hartford HealthCare is seeking a compassionate and dedicated Care Manager (LCSW) to join our Inpatient Psychiatry team at Charlotte Hungerford Hospital. In this part-time role, you will play a crucial role in providing timely evaluation and treatment to patients in psychiatric crisis in our Emergency Department. About Charlotte Hungerford Hospital: Charlotte Hungerford Hospital, located in Torrington, Connecticut, is a 122-bed general acute care community hospital serving the residents of Litchfield County and Northwest Connecticut. Details:

  • Job Schedule: Part-Time
  • Standard Hours: 8
  • Job Shift: Shift 1
  • Shift Details: Saturdays, Day or evening 8-hour shift. Three weekends per month.

Responsibilities:

  • Evaluate and provide initial treatment to patients in psychiatric crisis who present in the Emergency Department
  • Collaborate with the Emergency Department staff and on-call Psychiatrist to develop a care plan based on patient condition and available resources
  • Ensure patient satisfaction and maintain confidentiality while addressing age-specific patient needs
  • Work as part of an interdisciplinary team to provide support and clinical supervision

Qualifications:

  • Master's Degree in Social Work
  • State of Connecticut licensure as an LCSW
  • 1 or more years of experience providing crisis intervention services in a behavioral health setting
  • Experience with dual diagnosis, children, adolescents, and geriatric populations preferred
