Job Description:

As a Care Manager, you will be part of our collaborative practice model that includes patients, nurses, social workers, physicians, and other members of the healthcare team. You will utilize your clinical expertise and social work skills to determine and facilitate the most appropriate level of care for patients. Your responsibilities will include patient assessment, discharge planning, coordination of resources, and collaboration with physicians and healthcare team members.

Responsibilities:

  • Conduct comprehensive patient assessments, including psychosocial and financial aspects
  • Plan and coordinate patient care along the continuum
  • Collaborate with physicians and nursing staff to develop and implement treatment plans
  • Ensure customer satisfaction and maintain patient confidentiality
  • Effectively utilize medical software and technology

Qualifications:

  • Master's degree in Social Work from an accredited school
  • Active Licensed Clinical Social Worker (LCSW) licensure in the State of Connecticut
  • Minimum of 3 years of Medical Social Work experience, preferably in an acute care setting
  • Case Management certification (ACM or CCM) strongly preferred
  • Strong communication and interpersonal skills
  • Knowledge of the Social Work Process
  • Ability to manage multiple priorities and projects
  • Proficiency in Microsoft Office, Outlook, and medical software
  • Skilled in Motivational Interviewing
