Hartford HealthCare is seeking a dedicated and compassionate Care Manager (LCSW) in the Inpatient Psychiatry department. This is a full-time position with a Monday to Friday, 7:00 am - 3:00 pm shift. As a Care Manager, you will play a crucial role in our collaborative care management model, working alongside a multidisciplinary team to provide comprehensive patient care. Role and Responsibilities:

  • Collaborate with patients, nurses, social workers, physicians, and other healthcare team members to facilitate care along a continuum
  • Assess and plan the most appropriate level of care and identify discharge needs
  • Ensure effective resource coordination for optimal health outcomes
  • Collaborate with physicians and other healthcare professionals to develop and implement medical treatment plans
  • Maintain patient confidentiality and sensitivity to age-specific and cultural needs

Qualifications:

  • Master's in Social Work from an accredited school of Social Work
  • Active Licensed Clinical Social Worker (LCSW) licensure in the State of Connecticut
  • Minimum of 3 years of Medical Social Work experience, with acute care experience preferred
  • Case Management certification ACM or CCM strongly preferred
  • Strong communication and interpersonal skills
  • Knowledge of the Social Work Process
  • Ability to manage multiple priorities and projects within prescribed schedules and resources
  • Proficiency in Microsoft Office, Outlook, Meditech HCIS software, and web-based programs
  • Skilled in Motivational Interviewing
