Role and Responsibilities

As a Care Manager, you will:

• Conduct timely evaluations and provide initial treatment to Emergency Department patients in psychiatric crisis.
• Collaborate with the Emergency Department staff and on-call Psychiatrist in developing comprehensive care plans.
• Ensure customer satisfaction by maintaining patient confidentiality and meeting age-specific patient needs.

Qualifications

To be successful in this role, you should have:

• Master's Degree in Social Work
• State of Connecticut licensure as LCSW
• Experience providing crisis intervention services in a behavioral health setting
• Experience with dual diagnosis, children, adolescents, and geriatric populations is preferred
