As a Care Coordinator at Charlotte Hungerford Hospital, a 122-bed community hospital located in Torrington, Connecticut, you will have the opportunity to work in a small traditional New England community setting while benefiting from the resources and opportunities of a large health system.

At Charlotte Hungerford Hospital, we value personalized attention and utilize advanced technology and clinical partnerships to create a convenient, safe, and comfortable patient environment. Our Care Management process is a collaborative practice model involving patients, nurses, social workers, physicians, healthcare team members, caregivers, and the community. As a Care Coordinator, you will facilitate effective resource coordination and ensure access to care and appropriate utilization of resources for our patients. Your expertise in Case Coordination and your commitment to patient-centered care will be vital in achieving optimal health outcomes.

Qualifications

  • Masters in Social Work from an accredited school of Social Work
  • Minimum of 3 years of Medical Social Work experience, with Acute Care experience preferred
  • Current license as Masters in Social Work in the State of CT (LMSW)
  • Case Management certification ACM or CCM strongly preferred
  • Ability to effectively utilize Microsoft Office, Outlook, Epic software, Morrissey, and web-based programs
