As a Care Coordinator, you will facilitate effective resource coordination and ensure access to care and appropriate utilization of resources for our patients. Your expertise in Case Coordination and your commitment to patient-centered care will be vital in achieving optimal health outcomes.

Qualifications

• Masters in Social Work from an accredited school of Social Work
• Minimum of 3 years of Medical Social Work experience, with Acute Care experience preferred
• Current license as Masters in Social Work in the State of CT (LMSW)
• Case Management certification ACM or CCM strongly preferred
• Ability to effectively utilize Microsoft Office, Outlook, Epic software, Morrissey, and web-based programs
