About Charlotte Hungerford Hospital

Charlotte Hungerford Hospital, located in Torrington, Connecticut, is a 122-bed general acute care community hospital serving as a regional healthcare resource for the 100,000 residents of Litchfield County and Northwest Connecticut. Responsibilities

• Collaborate with patients, nurses, social workers, physicians, healthcare team members, caregivers, and the community to facilitate care coordination.
• Ensure effective communication and resource coordination to help patients achieve optimal health.
• Utilize a variety of software and tools to manage patient information and coordinate resources.

Requirements

• Master's degree in Social Work from an accredited school
• Minimum of 3 years of Medical Social Work experience, preferably in an acute care setting
• Current license as a Masters in Social Work in the State of CT (LMSW)
• Case Management certification (ACM or CCM) strongly preferred
• Proficiency in Microsoft Office, Outlook, Epic software, and web-based programs
