Located in Torrington, Connecticut, Charlotte Hungerford Hospital is a 122-bed, general acute care community hospital. We serve as a regional healthcare resource for the 100,000 residents of Litchfield County and Northwest Connecticut. As a Care Coordinator, you will be an integral part of our Care Management team. This collaborative practice model includes patients, nurses, social workers, physicians, healthcare team members, caregivers, and the community. Your role will encompass effective resource coordination and communication to facilitate care along a continuum. We strive to achieve optimal health, access to care, and appropriate utilization of resources, while respecting each patient's right to self-determination.

Responsibilities:

  • Coordinate and manage the care for assigned caseload, ensuring an appropriate level of services and resources are provided
  • Conduct assessments, develop care plans, and monitor patient progress
  • Collaborate with healthcare team members to advocate for patients' needs
  • Provide counseling and support to patients and their families
  • Utilize software programs such as Microsoft Office, Outlook, and Epic for documentation and communication

Requirements:

  • Master's in Social Work from an accredited school
  • Minimum of 3 years of Medical Social Work experience
  • Acute Care experience preferred
  • Current license as a Master's in Social Work (LMSW) in the State of CT
  • Case Management certification (ACM or CCM) strongly preferred
  • Proficiency in Microsoft Office, Outlook, Epic software, Morrissey, and web-based programs
