Summary

We are seeking a dedicated and compassionate Care Coordinator (LMSW) to join our Case Coordination team at Hartford HealthCare. In this role, you will work at Charlotte Hungerford Hospital in Torrington, Connecticut. Join us in providing exceptional care to the residents of Litchfield County and Northwest Connecticut.

As a Care Coordinator, you will be part of our collaborative care management model, working closely with patients, nurses, social workers, physicians, and the entire healthcare team. Your primary goal will be to ensure optimal health outcomes, access to care, and appropriate utilization of resources for our patients while respecting their right to self-determination.

  • Standard Hours: 40
  • Job Shift: Shift 1
  • Shift Details: Monday through Friday with scheduled weekends throughout the year

About Charlotte Hungerford Hospital

Charlotte Hungerford Hospital is a 122-bed community hospital located in Torrington, Connecticut. We serve as a regional healthcare resource for the residents of Litchfield County and Northwest Connecticut. As part of the Hartford HealthCare system, we offer the benefits of a large network while maintaining the close-knit community feel.

Responsibilities

  • Collaborate with the care management team to develop comprehensive care plans for patients.
  • Coordinate necessary resources and facilitate effective communication for the continuity of care.
  • Assess patients' social, emotional, and functional needs and provide appropriate support.
  • Conduct regular patient assessments and reassessments to monitor progress and make necessary adjustments to care plans.
  • Advocate for patients' rights to self-determination and ensure their preferences are honored.

Requirements

  • Master's in Social Work from an accredited school of Social Work.
  • Minimum of 3 years of Medical Social Work experience. Acute Care experience preferred.
  • Current license as Masters in Social Work in the State of CT (LMSW).
  • Case Management certification ACM or CCM strongly preferred.
  • Proficient in Microsoft Office, Outlook, Epic software, and other relevant programs.
